Sister Julienne fights to secure Sister Mary Cynthia's release from the mental institution while escalating tensions between the US and the Soviets threaten to plunge the world into nuclear warfare. Meanwhile, Valerie is shocked to discover that one of her patients, Nadifa, an expectant mother from Somaliland, has undergone a procedure known as "pharaonic circumcision" (more commonly known as female genital mutilation), a practice common in parts of Africa and the Middle East. When Nadifa goes into labour, things take a terrifying turn.
